Advanced Care Planning – The Best Gift You Can Give

Pathways, northern Colorado’s leading and largest provider of hospice, palliative and grief and loss services, is presenting a series of education opportunities this summer.

The first of the education series is entitled Advanced Care Planning – The Best Gift  You Can Give and will be held on Tuesday, July 21 at 3:00 p.m. at Pathways at 305 Carpenter Road, Fort Collins.

Advance care planning addresses the health care you would want to receive in a medical crisis – based on your values, preferences, and conversations with your loved ones. The health care decisions that have been expressed by an individual and written down serve as the instruction for future events and take off the onus of decision-making from the family and loved ones. 

Learn how to how to be in charge of your health care decisions and give direction to our loved ones and caregivers. These decisions that you make today will ensure you and your family a better tomorrow. 

Experts in Advanced Directives will lead a discussion and answer questions on how to record your healthcare directives. Copies of the 5-Wishes document will be provided – free of charge – with assistance on how to complete for your personal choices.
