Imagine a place that feels like home; comfortable, warm and inviting, filled with love and support. But this is no ordinary home. In this home, hospital-level care is available to relieve intense pain, manage symptoms, and provide compassionate, supportive care for you, your family member, or a friend.

Welcome to Pathways Inpatient Care Center, a new, beautiful space specifically built for the purpose of providing you and your loved ones the highest level of care.

The Inpatient Care Center serves Larimer and Weld counties, providing patients and families a choice for intensive inpatient care when being at home is not possible when the patient’s condition meets acute hospice care guidelines.

The Facility

The Pathways Inpatient Care Center (PICC) features:
  • a spacious lobby to welcome visitors
  • a non-denominational chapel and other areas for quiet reflection
  • a large communal kitchen allows families to prepare meals and eat together
  • gardens and walkways throughout the property that blend indoor and outdoor spaces
  • skylights and ample windows allow enjoyment of Northern Colorado’s 300 days of sunshine

The Pathways Inpatient Care Center is designed and built to seamlessly integrate healthcare and hospitality in a peaceful, home-like environment.

What to Expect

Patient Rooms

Each of the 12 rooms are large enough to accommodate groups of visitors and up to three family members who can choose to stay overnight. Bedbound patients can enjoy the outdoors from the comfort of their bed. In addition, two rooms are equipped with negative pressure ventilation, allowing for the safe care of patients with COVID-19 and other highly infectious illnesses.can

Family and Visitors

Visitors are welcome 24 hours a day with patient or family consent. Visitors are not allowed to smoke or consume alcohol. Pets may visit with prior approval of care center staff. In addition, family conferences to discuss care and receive counseling are available from the Pathways Hospice Care Team. Should you desire to discuss patient care in greater detail, or other more complex medical or social issues please speak directly with your Pathways Staff Nurse or Care Center Manager.

Inpatient Care Center Staff

The PICC operates every day of the year and is staffed with Registered Nurses, Certified Nurse Aides and additional Hospice Care Center staff including social workers, counselors, Chaplains, music therapists and volunteers to meet patient and family needs as they arise. The Pathways medical team of physicians and nurse practitioners are available daily to provide expert medical care to relieve pain and suffering from difficult symptoms. The Pathways team collaborates directly with each patient’s Primary Care Physician to coordinate goals of care and transition out of the Care Center if their condition stabilizes.

Most caring, kind, compassionate staff. My dad was able to pass peacefully at the beautiful 12 bed facility in Fort Collins. He was given respect as a veteran, and the care for families while spending last days and moments with loved ones was over the top comforting!

Absolutely wonderful place. I was here while experiencing my grandfathers passing. The facility is beautiful, so asked if I could photograph it. They agreed and I uploaded the images to this page. My goal is to show others what a serene and gorgeous place their family member is going to, and that hopefully they find some comfort in that thought.

The staff is amazing and friendly. I really appreciated all the kind words and friendly smiles. They were willing to give a hug or two as well. My husband died last week. We were fortunate enough to transfer him to pathways inpatient center. We tried home, hospice for a few days, but it was really difficult for us. Once he got to hospice inpatient, his body relaxed with the great caregivers. He was able to be comfortable and have a peaceful death. We are forever grateful. Our son said we need bumper stickers that say “get me to pathways”! It brought so much peace to all of us and thank you once again to the staff
