President

Evan Hyatt

With over 20 years of experience in healthcare leadership, administration, operations, marketing and communications and fundraising and development communications, Evan Hyatt is the President of Pathways.

Hyatt previously served as CEO/president of Pathways from 2008 through 2016, and is re-joining the organization following a role as the Vice President of Marketing and Development at Care Synergy, Pathways’ supporting organization.

Hyatt began his career with Pathways as its marketing manager in 2001 when the organization was known as Hospice of Larimer County.  At the end of 2016, he left Pathways to become the Executive Director of Operations for Kaiser Permanente in Northern Colorado.  Then, following a brief stint with Banner Hospice, Hyatt joined Care Synergy in 2019.  Altogether, he has more than 18 years of experience in hospice and palliative care at Pathways and Care Synergy.

In the role of president, Hyatt will be responsible for the day-to-day operations of Pathways as well as collaborating with its board of directors to guide the mission, strategic objectives and growth of the organization. 

Medical Director

Dr. Nathaniel Chappelle

Dr. Nathaniel Chappelle stepped into the Medical Director role at Pathways after a year of serving as one of Pathways’ staff physicians. 

Prior to joining Pathways in 2023, Dr. Chappelle worked in a wide variety of settings. He practiced primarily rural medicine for a most of his career prior to joining Pathways. He has served as a primary care physician in many parts of rural Colorado while also covering emergency departments, rounding in rural hospitals, caring for long term care residents. 

More recently, he provided primary care services at a federally qualified healthcare center locally in Northern Colorado where he enjoyed caring for patients from many different backgrounds and with widely varying needs. 

Dr. Chappelle enjoys working on systems-based practice and looks forward to helping Pathways continue to provide excellent care for our patients.

Director of Clinical Services

Angie Sickmon

Angie Sickmon is a registered nurse and has been an integral part of the Pathways team for more than 17 years. Starting out as a nurse, she has filled many different positions at Pathways and now holds the position of Director of Clinical Services. Angie is first and foremost patient-focused, which has earned her the respect of her peers and the appreciation of the patients and families in hospice care. She has a strong commitment to customer service, and her prior nursing training enables her to bring tremendous insight into the special circumstances that may arise, as well as being a strong support to her team.

Director of Business Development

Jacque Taylor

Jacque Taylor was promoted into the position of Director of Business Development after serving as a Community Liaison at Pathways starting in 2019.

Jacque has an extensive background in sales leadership and management. Her career in sales started in 1992 for the Burke family at Mister Neat’s Formalwear

In 2005, Jacque and her husband, Mike, purchased and operated “Vern’s Place” in LaPorte. From there she went to the Silver Grill in Old Town, Fort Collins for a few years.  Being in restaurants helped her learn she loved the people not some much the food.

Jacque has a passion for training and building relationships with people.  This led her back to Burke Cleaners where she served as the Vice President of Sales and Marketing.

In 2018, Burke Cleaners was sold and this allowed her to move on and bring her skills to a place where she found purpose and a great culture – Pathways.

Chief medical Officer

Dr. Pradeep Rai

Dr. Pradeep Rai, MD, joins Care Synergy as Chief Medical Officer. With over a decade of healthcare leadership, Dr. Rai will oversee clinical operations across Care Synergy’s hospice, palliative care, and home health affiliates, including TDH, Pikes Peak Hospice, Pathways Hospice, and CVNA. Dr. Rai’s credentials include being a Diplomate of the American Board of Addiction Medicine and certifications from the National Registry of Certified Medical Examiners and the American Academy of Addiction Psychiatry. Actively engaged with medical associations, Dr. Rai contributes to healthcare standards through involvement with the American Medical Directors’ Association and the Colorado Medical Directors’ Association. With specialized training in burn surgery, Dr. Rai’s appointment underscores Care Synergy’s commitment to clinical excellence and compassionate care. Care Synergy’s President and CEO, Tim Bowen, expresses gratitude for Dr. Rai’s leadership experience, which uniquely qualifies him to lead the exceptional team of physicians and nurse practitioners.
