Pathways in Weld County

On track with being northern Colorado’s preferred provider of hospice and palliative care services, Pathways has had a presence in Greeley since 2017. We opened our current Greeley office in December 2020. We are steadily growing and adding services to our Weld county hospice branch to ensure everyone has access to quality end of life care.

Services

Grief & Loss

In 2019, we have added grief and loss programs to our Weld county location. For the summer session, our caregiver community groups will be occurring the fourth Monday of the month. See our events calendar for upcoming classes and groups to attend.

Hospice Care

Greeley hospice care is available in many forms through Pathways. We offer in-home care, assisted living care, and veteran care.

Palliative Care

For those in Greeley and Weld county looking for palliative care, Pathways provides specialized service for pain management, counseling, support, and guidance. Our palliative care services are available for those dealing with an ongoing illness, children with life-limiting illnesses, and those suffering from dementia.

Veterans Care

Pathways is a member of the “We Honor Veterans” project of the National Hospice and Palliative Care Organization. We honor Weld County veterans through specialized care with our companionship volunteers and recognition teams.

Pathways relies on the generous hearts of many volunteers. In 2018, the Weld branch had grown enough to warrant its own group of volunteers.
