Pathways, located in Larimer and Weld county, is a local nonprofit providing symptom management and support for serious long-term illness (Palliative Care), expert medical and hospice care for individuals navigating the last months of life, and grief counseling and programs for those mourning the loss of a loved one or dealing with a tragedy. From our beginnings in 1978, when a group of nurses came together in Loveland to voluntarily care for a friend in the final months of her life, we’ve been caring for Northern Colorado residents regardless of financial circumstance.

We serve individuals living in Larimer County, western Weld County, and the Highway 85 and I-25 corridors including the towns of Fort Collins, Loveland, Greeley, and Windsor.

Our mission is to provide compassionate, excellent, comprehensive community care in Larimer and Weld Counties for those who have an advanced medical condition, serious illness and those who are grieving. If you, your family or friends should ever need our hospice, palliative and community care, we’re here to help you.

Pathways is proud to offer specialized services for military veterans and their families. We hold the highest Level IV certification in the We Honor Veterans program, demonstrating our dedication to honoring those who have served our nation.

  • Partnering with community veteran organizations.
  • Offering a veterans-to-veterans volunteer program.
  • Recognizing the contributions of veterans through ceremonies and events.
  • Providing resources through our community library specifically for veterans.
  • Identifying patients and immediate family members who have served our country.
  • Paying tribute to all US military veterans through our on-site Honoring Veterans Garden.
  • Working collaboratively with Larimer County Veterans Services and the VA Medical Center.
  • Educating our clinical professionals to provide specialized care to patients with a history of military service and possibly physical or psychological trauma.
