The Pathways philosophy is to live every moment fully, with dignity and comfort. Hospice is for any patient, of any age, who has been identified with a terminal diagnosis that has a life expectancy of six months or less. We make care giving convenient for patients and their families by coming to them–whether it is at their home, skilled nursing facility, assisted living facility or independent living facility.
Our team approach is adapted to both the patient and their family to provide personalized medical attention and comfort to all involved. Ultimately, hospice care is focused on the patient’s wishes and goals. We focus on assisting our patients in living out the last chapter of their life on their terms.
Pathways’ Grief and Loss Team steps in after the loss of a loved one to help family and friends cope with the overwhelming feelings that come from loss. We understand everyone grieves differently so we offer support in the form of individual or group counseling, phone calls and creative therapy.
