Strengthening Healthcare Partnerships: Pathways Visits UCHealth Hospitals
Providing compassionate, coordinated care takes teamwork. At Pathways Hospice, we value our partnerships with healthcare professionals throughout Northern Colorado who help ensure patients and families have access to the support and services they need.
Over the next several weeks, Pathways will participate in resource fairs at UCHealth hospitals, connecting with clinical staff and other key healthcare partners with whom we regularly collaborate to care for patients.
Last week, Pathways nurses Jessica Moen and Yvonne Willemann represented our team at Poudre Valley Hospital in Fort Collins. The event offered an opportunity to connect with healthcare professionals, share information about Pathways' services, and strengthen the relationships that support collaborative patient care.
These connections are an important part of our work. By staying engaged with our healthcare partners, we can help providers better understand hospice care and how Pathways can support patients and their loved ones during challenging times.
